Nonconforming Material, Failure Investigation and CAPA
Containment, disposition, investigation and corrective action are connected decisions with different purposes. A nonconforming-material record controls affected material. A failure investigation examines what happened and why. CAPA addresses the causes and checks whether the chosen action works over time.
Control the affected product first
Identify the discrepancy, affected items and acceptance status. Protect against unintended use while authorized personnel assess disposition. Record the basis for decisions and any required reinspection or additional review. Preserve the evidence needed for investigation before material is reworked, returned or discarded.
Test causes and action effectiveness
Use observations, records and targeted checks to distinguish a supported cause from a convenient explanation. Assess whether the issue extends to other products, processes or periods. Choose actions that address the cause and define how effectiveness will be evaluated. Closing the action after implementation alone leaves the central question unanswered: did the action reduce recurrence or otherwise achieve its objective?
Practical checklist
Identify and contain affected material.
Record authorized disposition and rationale.
Preserve investigation evidence.
Assess the extent and supported cause.
Define and evaluate effectiveness.
Example
Replacing a damaged gauge may correct one immediate problem. The investigation can also ask how the damage went undetected and whether other inspection decisions were affected. An effective action may need changes to checks, storage or calibration controls.
Common mistakes
Avoid defaulting to operator retraining without evidence of a competency cause. A completion signature proves the training occurred; it does not show that the underlying process problem has been resolved.
Choose the relevant A.P. Lyon documents
Use nonconforming-material controls to identify, segregate, review and disposition discrepancies.
Use failure investigation to examine failures against performance specifications and document the supported root cause.
Use CAPA documentation for action planning, implementation, effectiveness review and closure.
Review the product page, document previews and any content list before choosing. Adapt the selected documents to your device, processes and applicable requirements; the organization must generate and review its own implementation evidence.
